Job Title: Social Worker - Care Coordinator

Duties & Responsibilities: The Care Coordinator Social Worker is responsible for managing and coordinating the delivery of comprehensive care for patients, ensuring that their social, emotional, and healthcare needs are met effectively. This role involves working closely with patients, families, and interdisciplinary teams to develop and implement care plans, address barriers to care, and ensure continuity of services. This is a 13 week assignment working 5 8 hour shifts or 4 10 hour shifts per week with weekend & holiday rotation.

Minimum Requirements:

• Georgia Social Worker license
• BLS (AHA)
• 2 years of experience in healthcare in acute care setting, related field or skilled care environment or community-required
• Background in medical social work in an acute care setting-preferred
