Description: Our client is currently seeking a Case Manager RN in Medford, Oregon!

Job Title: Case Manager RN

Job Type: Contract to Hire

Location: Medford, Oregon 97501

Hours: 8:00 AM - 5:00 PM (Monday - Friday)

Rate: Competitive and Dependent on Experience

Primary Responsibilities:

• Ensure accurate and complete documentation.
• Communicate and collaborate with the interdisciplinary care team.
• Assist the provider/team with various care coordination activities.
• Assist the provider/team with benefit determination associated with Medicare Part A, Part B, and Part D benefits.
• Collaborate with the primary advanced practice clinician (APC) and facility to identify and respond to patient changes in condition.
• Assist the provider/team with therapy coordination.
• Participate in facility-based meetings.
• Schedule and participate in family conferences, team meetings, and team case presentations.
• Coordinate training, tracking, and compliance with quality measures.
• Assist in creating and conducting in-services for selected audiences.
• Assist providers in ensuring complete and accurate documentation and coding, medication reconciliation, nursing rounds, assessment, and patient documentation.
• May write verbal orders from the provider in the chart in compliance with state RN practice laws and individual nursing facility practices.
• Assist in managing members through acute medical situations or exacerbations of chronic conditions, including testing, diagnostics, and interventions.
• Assist in managing members during short-term sub-acute rehabilitation episodes.

Required Qualifications:

• Active and unrestricted license in the state of residence, or ability to obtain by start date.
• Associate degree or higher in Nursing (RN).
• Certified in Basic Life Support.
• Ability to complete physical requirements, including lifting a 30-pound bag and performing physical assessments.
• Access to reliable transportation & ability to travel between facilities.
• Proficient computer skills, including the ability to document medical information with written and electronic medical records.
• Effective communication skills and time management.
• Ability to work with diverse care teams in a variety of settings.
• Proof of full COVID-19 vaccination prior to employment.
• Candidates are required to pass a drug test before beginning employment.

Preferred Qualifications:

• 1+ year of clinical experience in practice (long-term care setting preferred).
• Experience in meeting the medical needs of patients with complex behavioral, social, and/or functional needs.
• Understanding of geriatrics and chronic illness.

Travel Details:

• Location: Medford, OR- including Jackson and Josephine Counties
• Timing: During shift hours
• Facilities: Traveling between 2-3 facilities
