Sandra Eskenazi Mental Health Center, Indiana's first community mental health center, provides comprehensive care for emotional and behavioral problems, including severe mental illness and substance abuse. The Sandra Eskenazi Mental Health Centeroffers both inpatient and outpatient services, including several outreach centers as well as clinic- and community-based services.

Job Role Summary

The CCBHC Navigator assists clients and their families seeking Certified Community Behavioral Health Clinic (CCBHC) services at Sandra Eskenazi Mental Health Center (Sandra Eskenazi MHC) navigate the CCBHC process, barriers, service offerings, and additional information or resources that are available at Eskenazi or in the community. The CCBHC Navigator assists individuals transitioning between different treatment programs and especially between different levels of care, provides mutual support and self-help groups, peer support for special populations, peer education and leadership development, and peer recovery services. The CCBHC Navigator may additionally support in immediate Social Determinants of Health needs, such as financial or housing needs. This role supports Eskenazi Health's engagement and partnership with the community surrounding the CCBHC aligning with the mission, vision and values of Eskenazi Health (EH) and strategic plans of EH and Sandra Eskenazi MHC. The CCBHC Navigator connects the client to Eskenazi programs, helping to navigate between departments, and providing specialized service to address the unique needs of the client.

Essential Functions and Responsibilities

  • Embraces, understands, and operates under the Recovery Model, including AIDET (Acknowledge, Introduce, Duration, Explanation, Thank You) and the spirit of motivational interviewing.
  • Uses excellent customer service to provide unique client-focused solutions to address problems.
  • Attempt to solve immediate Social Determinant of Health needs via in house supplies or by connecting to community resources.
  • Expedites community connectedness for clients by providing warm handoffs to a trusted community-based organizations with follow-up.
  • May be asked to assist CCBHC Liaisons and treatment teams in coordinating transitions of care for clients between different levels of care.
  • Helps connect CCBHC clients to peer related services, such as mutual support and self-help groups; peer support for special populations; peer education and leadership development; and peer recovery services
  • Works closely with CCBHC treatment teams including mobile crisis, with the goal of clear communication and timely follow up to ensure best client outcomes and satisfaction.
  • Supports staff leadership in providing community outreach as needed at health fairs and comparable events.
  • Will use designated electronic health record to document and record client interactions, including sharing of information with primary and other relevant treatment teams.
  • Meets Sandra Eskenazi Mental Health Center requirements for continuing education in accordance with best practice, CCBHC, Joint Commission, and other regulatory standards.

Job Requirements

  • High School diploma or equivalent required
  • Peer Recovery certification required
  • Minimum of 2 years in Recovery required
  • Minimum of 1 year experience providing peer recovery services
  • Valid Indiana driver's license and driving record that meets Eskenazi Health's driving policy requirements

Knowledge, Skills & Abilities

  • Demonstrates effective communication skills; ability to use age appropriate communication skills
  • Understands and has ability to communicate effectively with individuals from diverse backgrounds-especially clients with substance use
  • Strong customer service skills
  • Knowledge of and proficiency in operating computer programs including Microsoft Office
  • Ability to prioritize tasks and work effectively with complex tasks under pressure/time restraints/irregular schedules
  • Ability to effectively address issues of ethnic and economic diversity
  • Strong interpersonal skills and a genuine caring attitude for clients, families and staff
  • Detail-oriented with the ability to think critically and independently
  • Effective written and verbal communication skills, including ability to communicate in a professional manner with physicians, administration and the general public
  • Excellent organizational skills with the ability to prioritize work and devise own work patterns/methods; flexible to changing priorities; ability to work with minimal supervision

Sandra Eskenazi Mental Health Center's primary mission is to serve individuals with serious mental illness and chronic addiction as well as seriously emotionally disturbed children and their families.
