Cincinnati Children's Hospital Medical Center is seeking a skilled and detail-oriented individual to join our Health Information Management (HIM) team as a Representative II. In this role, you will play a vital part in ensuring the accuracy and integrity of patient records by preparing statistical reports, coding diseases and operations in accordance with standardized classifications, and maintaining indexes following established plans and procedures. This position requires a solid foundation of skills and knowledge acquired through formal training or work experience. You will handle routine tasks with the ability to interpret and resolve issues under moderate supervision, contributing to the efficient and compliant management of health information.
JOB RESPONSIBILITIESPatient Records Management
- Collect and reconcile discharged patient records from inpatient units and same-day surgery.
- Retrieve outpatient records from clinical areas and designated locations.
- Prepare documents for imaging following established procedures, including sorting documents based on prepping protocols.
- Convert and manage electronic images using scanning technology to produce high-quality digital records.
- Upload images into the Imaging system after completing a quality review.
- Identify and resolve issues related to imaging processes through discussion and problem-solving.
- Prepare boxes of imaged paper records for offsite storage, including creating detailed inventory lists.
- Create accounts in ChartMaxx for clinic shadow records requiring special account creation.
Customer Service
- Assist CCHMC staff and customers with patient information requests, adhering to HIPAA and HIM standards.
- Prioritize and troubleshoot requests for information, ensuring appropriate release per CCHMC policies.
- Demonstrate flexibility in assignments to meet customer needs.
- Collaborate as an effective team member within individual and departmental teams.
- Coordinate requests for information related to research, payment, and healthcare operations.
Communication and Support
- Effectively communicate with internal and external customers, promptly addressing inquiries via phone or in person.
- Research and respond to information requests, following defined protocols.
- Act as a resource on electronic content and record management policies and processes.
- Serve as a liaison, working collaboratively with other departments, external agencies, and vendors.
- Troubleshoot and propose solutions for identified problems.
- Assist in onboarding new staff and orient them to departmental processes.
New Hire Training
- Train newly hired HIM staff on all aspects of the Enterprise Imaging Process, policies, and procedures.
- Provide secondary quality reviews of work by new hires during their initial work period.
- Offer support, guidance, and feedback to management on training progress.
- Perform equipment checks and basic troubleshooting to ensure operational functionality at the start of each shift.
Quality Review
- Conduct secondary quality reviews of scanned documents, including those ready for destruction.
- Ensure the organization and maintenance of medical records in all formats.
- Maintain production and quality statistics, consistently meeting departmental standards.
- Organize and maintain a safe, efficient, and accessible workspace for employees and patients.
JOB QUALIFICATIONS
- High school diploma or equivalent
- 2+ years of work experience in a related job discipline
