The Manager Social Work Case Management is responsible for managing a team of social work case managers engaged in performing social work case management services by making integrative psychosocial assessments, developing, implementing, coordinating, monitoring, and evaluating social work goals designed to optimize member access to social services across the care continuum and ensuring member access to services appropriate to their psychosocial needs.
How you will make an impact:
• Ensures that appropriate psychosocial assessments are conducted to identify individual needs and a specific social services/psychiatric needs to address objectives and goals as identified during assessment.
• Manages the implementation of social work goals by advocating for member needs. Networks with other community resources.
• Leads the development and implementation of a metric system to track baseline levels of functioning throughout course of social work services.
• Manages the identification and recommendation of specific psychosocial resources to meet members identified needs.
• Manages and evaluates the effectiveness of social work goals.
• Interfaces with Registered Nurse Care Managers and Management on the development, implementation, and progress of social work goals, and the members responsiveness to resources rendered.
• Manages the evaluation of members' ability to independently manage self and the locating of alternative resources when limitations are identified.
• Oversees the guidance provided to members seeking alternative solutions to specific social, cultural or financial problems that impact their ability to manage their healthcare needs.
• Develops strategies to support healthcare needs and manages the implementation of plans in support of case decisions.
• Manages the facilitation and coordination of behavioral health resources as individual member needs are identified.
• Manages social work case audit activities. Hires, trains, coaches, counsels, and evaluates performance of direct reports.
Minimum Requirements:
• Requires MS or PhD in Clinical Social Work or related field and minimum of 7 years of clinical experience; or any combination of education and experience, which would provide an equivalent background.
• Current license as a Clinical Social Worker (LCSW) in applicable state(s) required.
For candidates working in person or remotely in the below locations, the salary* range for this specific position is $107,300 - $154,240
