Location: Long Branch, NJ
Description: Our client is currently seeking an Advanced Practice Clinician (Nurse Practitioner/Physician Assistant) in Ocean County and Monmouth County, NJ to join their team at a Senior Community Care (SCC) program. The APC within the SCC will provide care to the highest-risk health plan members as part of an interdisciplinary team.

Work Schedule: Monday to Friday, 8 am to 5 pm; No on-call, weekends, or holidays.

Responsibilities:

• Conduct comprehensive assessments.
• Manage acute and chronic medical and behavioral conditions effectively.
• Ensure accurate and complete documentation, including ICD 10 conditions.
• Collaborate and communicate with the interdisciplinary care team.
• Conduct advanced illness and advanced care planning conversations.
• Provide counseling and education to patients and caregivers.

Qualifications & Requirements:

• Active and unrestricted Family, Adult, Gerontology, or Adult Gerontology Acute Care, license in the state of residence, or ability to obtain by start date.
• Education:
• NP: Graduate of an accredited Master of Science Nursing or Doctor of Nursing Practice program.
• PA: Graduate of an accredited Physician Assistant degree program.
• Active Nurse Practitioner or Physician Assistant certification through a national board.
• Current, active DEA licensure/prescriptive authority or ability to obtain post-hire, per state regulations.
• Access to reliable transportation
• Ability to gain a collaborative practice agreement.
