As a Physical Therapist, you'll assess, plan, organize, and participate in rehabilitation programs designed to enhance mobility, relieve pain, build strength, and prevent deformities in patients recovering from injury or illness. You'll deliver services across the care continuum with a focus on quality, cost-efficiency, and customer satisfaction, while ensuring compliance with all regulatory standards.
Core Responsibilities
Patient Assessment

  • Collaborate with physicians to establish and evaluate treatment programs, care plans, goals, and discharge plans tailored to each patient's needs.
  • Develop a personalized plan of care with input from patients and their families.
  • Create realistic discharge plans that incorporate the goals of the patient and their family.

Treatment Planning and Provision of Care
  • Implement the physical therapy treatment plan, selecting activities that support patient progress.
  • Adjust goals and plans based on patient and family feedback.
  • Provide ongoing education and training to patients and families.
  • Deliver discharge instructions and referrals to community resources as needed.
  • Serve as a resource for staff, families, and physicians regarding rehabilitation services and equipment.

Documentation
  • Document evaluations and plans of care in accordance with departmental guidelines.
  • Record each treatment session following established protocols.
  • Complete and submit all required documentation, including family education and discharge information, within organizational guidelines.

  • Meet core and annual competencies according to departmental timelines.
  • Fulfill all educational requirements to maintain licensure.
  • Participate in the orientation and training of new employees.
  • Complete all mandatory hospital and system requirements.

Required Minimum Education
  • Bachelor's, Master's, or Doctorate in Physical Therapy.

Required Licenses and Certifications
  • Licensed Physical Therapist.
  • Basic Life Support (BLS) certification (Instructor preferred).

Required Minimum Skills
  • Strong communication skills with a focus on customer service.
  • Team-oriented mindset.
  • Basic computer skills.
