Case manager for MGH's expanding cellular therapy program. Be part of a program offering the newest treatments for cancer patients and new indications in clinical trials for non-oncology diseases. Collaborate with nurse navigators, social workers, physicians and research nurses to support patients and their caregivers through a complex treatment journey.

Job Summary
Act as a patient advocate ensuring the cellular therapy patient receives the necessary assistance to be able to follow through with the personal treatment plan. Facilitates with the provider, patient and family to access the health care resources and health insurance. Makes contacts and referrals for social support, networks and VNAs.

Reviews patient's insurance and meets with patient/family to review for financial toxicity and options available to patient. Communicates with Patient Financial Services (PFS) to assist and support patient with insurance coverage. Navigation and application for any funding resources available to specific diagnosis

Must communicate effectively and works closely with interdisciplinary care teams including referral management teams and referrals for case management programs. In collaboration with the Interdisciplinary Team, monitors the patient's progress and plan of care with the aid of internal and external utilization and quality guidelines.

Identifies, documents, and reports issues and system barriers Develop a cohesive and strong team-oriented relationship with oncologists, nurses and other health care professionals. Work in partnership with interdisciplinary healthcare team members to facilitate the best in patient care.

Work with social work, manufacturers and clinical trial support programs and local resources to facilitate nearby lodging, transportation and food assistance when patient needs to stay in close proximity to hospital for treatment and follow-up. Monitors the patient's progress, intervening as necessary and when appropriate to ensure that the plan of care and services provided are patient focused and friendly, high quality, efficient, and cost effective.

Utilizes a translator when appropriate and incorporates awareness of the patient's cultural needs that may impact the plan of care Collaborates with Utilization Management Department to determine Level of Care (LOC) When patient may require rehabilitation services during treatment phase, case manager will assist with review of options and facilitate as part of the care team.

Assists with transportation issues (PT1 through Mass Health, the Ride, parking at MGH, the shuttle to and from OCB, etc.) Responsible when asked by oncology providers to coordinate, setup and follow health care services needed in the home.

Perform prior authorizations and retrospective reviews on any medications that have been labeled as needing a PA. Refer PA's back to the oncologist when the request does not meet the medical necessity guidelines or a peer to peer review is necessary. Training and sharing information with existing Cancer Center Case Managers and Social Services Department

Qualifications

Registered Nurse (RN) license in Massachusetts Bachelor of Science in Nursing (BSN) Experience in case management Strong clinical knowledge and experience in the oncology setting preferred Excellent communication and interpersonal skills Ability to work independently and as part of a team Strong organizational and time management skills
